How to take the photos
A plain phone camera is enough. Four photos, taken in good light, straight into the lens.

Find good light
Daylight from a window works better than an overhead light. Face the light rather than have it behind you.

Smile straight on
Look straight at the camera and smile so your teeth show.

Cheeks pulled back, left and right
Two photos: gently pull one cheek aside with a finger or a spoon, then the other, so the back teeth show on each side.

Upper and lower arches
Two more photos, looking down into a mirror or with someone else holding the camera: the upper teeth from below, then the lower teeth from above.
