Growth modification
Appliances that guide how the jaws grow, used while a child is still growing. The aim is a better fit between the upper and lower jaw now, so later treatment has less to correct.
Who it suits
Kids and early teens whose upper and lower jaws are not growing in step: a lower jaw that sits too far back, an upper jaw that is too narrow or too far forward, or a crossbite that comes from the jaw rather than a single tooth. It only works while growth is still happening, which is why the timing is checked from age 7.
How it works

Records at the consultation: photographs, a scan, and X-rays where needed, to see how the jaws are growing.

Dr. Mashaly chooses the appliance for the case: a functional appliance that holds the lower jaw forward, an expander that widens a narrow upper jaw, or a headgear-type appliance that steers the growth of the upper jaw.

The appliance is fitted and adjusted at regular visits while the jaw grows.

Once the jaws are in a better position, the result is held, and any braces or aligners still needed follow in the teenage years.
Timing
It depends on the appliance and on how your child grows. You get an estimate for your own case at the consultation, after the records are taken.
The first week
- A new appliance feels large for the first few days. Speech and swallowing settle within about a week.
- After an expander is turned, a feeling of pressure across the nose and cheeks is normal. It fades within a few days.
- More saliva than usual at the start. It passes.

Day-to-day care instructions are shared privately with each patient, matched to their own treatment.
Questions parents ask
- Is my child too young for this?
- Not usually. Growth modification depends on the jaw still growing, so it is done in kids and early teens. The first check at around 7 tells you whether anything is needed yet.
- Does it replace braces later?
- Sometimes it reduces what is needed later, sometimes not. Many children still need a second phase with braces or aligners once the adult teeth are in. You will hear which is likely at the consultation.
- Is it worn all day?
- It depends on the appliance. Some are fixed in place, others are worn part of the day and at night. The schedule is set at the fitting.
- What if we wait?
- Once growth slows down, the same jaw difference can only be treated by moving the teeth, and in some adult cases with jaw surgery.
