Habit-breaking appliances

Some bites are shaped by a habit rather than by the teeth themselves. A small appliance helps a child let go of the habit, so the teeth and jaws can develop as they should.

Who it suits

Mainly kids whose habit is already affecting the teeth or the bite: thumb or finger sucking past the age when most children stop, a tongue that pushes forward between the front teeth when swallowing, or breathing through the mouth most of the time. Common signs are an open bite, upper front teeth tipping forward, or a narrow upper jaw.

How it works

    At the consultation, the habit and its effect on the bite are checked, and both the child and the parent are asked about it.

    The appliance is chosen for the habit: a guard that makes thumb or finger sucking hard to keep up, a tongue crib or tongue guide that trains the tongue to rest in the right place, or an oral screen that encourages the lips to close and breathing through the nose.

    Where mouth breathing comes from a blocked nose or large tonsils, referral to an ear, nose and throat specialist comes first, alongside the orthodontic plan.

    Once the habit stops, the teeth often start to settle on their own. Anything left is treated later with growth modification, braces or aligners.

Timing

It depends on the habit and on the child. The appliance stays until the habit has stopped and the bite has had time to respond. You get an estimate at the consultation.

The first week

  • Eating and speaking feel different for the first few days while the tongue gets used to the appliance.
  • The first nights are usually the hardest for a thumb or finger habit. It gets easier as the habit fades.
  • Message us if anything rubs or feels loose.

Questions parents ask

When should a thumb habit worry us?
Most children stop on their own by the time the adult front teeth come in. If the habit carries on after that, or the bite is already changing, it is worth a check.
Does the appliance hurt?
No. It makes the habit harder to do and less satisfying, without pain. Children usually get used to it within a few days.
Does it work if my child does not want to stop?
It works best when the child wants to stop. Dr. Mashaly explains the appliance to the child as well as the parent, in words the child understands.
What about mouth breathing?
Mouth breathing often has a cause in the nose or throat, so that is checked first. An appliance supports breathing through the nose once the airway allows it.
